Creating a Blank Application Form
The questions in the Application Form are dynamically populated, meaning that new questions are generated as you respond to earlier questions. If you would like to see all of the application questions at once, you can create a blank form for any type of application.
Blank application for a sustainable community plan
- Answer only the questions identifying your type of organization in Part 1, Section A) Applicant Information.
- Click the Next button for the subsequent questions without answering them until you get to Part 1, Section B) Information About Your Initiative.
- Answer only the questions identifying the type of initiative you are undertaking in Part 1, Section B) Information About Your Initiative.
- Click the Next button for the subsequent questions without answering them until you get to the Take me to Part 2: Purpose and sustainability impacts button and click on it.
- Click the Next button for the subsequent questions without answering them until you get to Part 2, Section B) Environmental Indicators and Benefits.
- Click the Next button for all remaining questions without answering them until you get to Part 5, Declaration. This is the end of the Application Form.
- Save your new blank form.
Blank application for a feasibility study or a combination feasibility study-field test
- Follow steps 1 to 5 listed above for creating a blank form for a sustainable community plan.
- Select and add the type of indicators you will use to evaluate the impacts of your environmental initiative and click the Next button. By clicking the Next button, the tables and descriptors of the indicators you selected will be generated.
- Click the Next button for all remaining questions without answering them until you get to Part 5, Declaration. This is the end of the Application Form.
- Save your new blank form.
Blank application for a field test
- Follow steps 1 to 5 listed above for creating a blank form for a sustainable community plan.
- Click the Next button for the subsequent questions without answering them until you get to Part 1, Section C) Prerequisites.
- If you are required to complete an environmental assessment of your initiative, answer only the questions associated with this and click the Next button for the subsequent questions without answering them until you get to the Take me to Part 2: Purpose and sustainability impacts button and click on it.
- If you are not required to complete an environmental assessment of your initiative, click the Next button for the subsequent questions without answering them until you get to the Take me to Part 2: Purpose and sustainability impacts button and click on it.
- Click the Next button for the subsequent questions without answering them until you get to Part 2, Section B) - Environmental Indicators and Benefits.
- Select and add the type of indicators you will use to evaluate the impacts of your environmental initiative and click the Next button. By clicking the Next button, the tables and descriptors of the indicators you selected will be generated.
- Click the Next button for all remaining questions without answering them until you get to Part 5, Declaration. This is the end of the Application Form.

- Answer only the questions identifying your type of organization in Part 1, Section A) Applicant Information.
- Click the Next button for the subsequent questions without answering them until you get to Part 1, Section B) Information About Your Initiative.
- Answer only the questions identifying the environmental initiative you are undertaking and the questions related to borrowing money from FCM.
- Follow steps 2 to 6 listed above for creating a blank form for a field test.
